Berthoud is two kinds of town at once when it comes to concrete, and which one you are in changes what is wrong with your slab.
In town, the housing stock is older and the concrete with it. Decades of foundation backfill, utility trenches and tree roots have had time to work, and settlement here is gradual and usually shows first at driveway approaches and front walks.
Around the edges, new development sits on graded fill that is still compacting, which produces the same young concrete settlement we see in Windsor and Timnath.
Running through both is irrigation. Berthoud sits in ditch country, and between agricultural water, lawn irrigation and older sprinkler systems, there is plenty of water available to move soil out from under a slab. Washout at slab edges near downspouts and sprinkler heads is one of the most common causes we find here.

Part of that visit is working out why it moved. Downspouts, irrigation, grade and drainage all get checked, because if the cause is still active a lift will not hold and you deserve to know that before you spend anything.
